A Passport backup includes your visited courses, visit dates, ratings and notes. Keep the file somewhere safe, such as Files or iCloud Drive. Create a backup before changing phone, removing the installed app or clearing browser/site data.

Installing the Home Screen app starts with separate storage on iPhone. Back up your Passport in Safari, then restore that backup in the installed app.
